The Child Health program provides home visits and case management as needed for all babies born through maternity

clinic and high-risk infants referred by private providers. The program also includes:

• Immunization services for children
• Case management for Children with Special Health Care Needs

This program provides registered nurse case management for children with special health care needs by securing

services and resources for them and serving as a liaison for home, school, physician and other agencies/programs.

Referrals are accepted from physicians, schools, and parents. There is NO fee for this service.
