The Individuals with Disabilities Education Act (IDEA) requires that transition services be included in every student's IEP beginning at age 16 or earlier, when appropriate.  "Transition services" refers to a coordinated set of activities for a child with a disability. These activities are designed to improve academic and functional achievement and to facilitate the child's movement from school to post-school activities.  The Louisiana Department of Education strives to help youth with disabilities and their families achieve desired post-school outcomes through student-driven transition planning, interagency collaboration, technical assistance and training.
